How should a team measure whether it is actually executing?
Chapter 12 makes output the final arbiter of execution: not intention, effort, activity, plan or sincerity. Did the thing get delivered, to specification, on time, at acceptable quality and within budget? That is why the Check stage opens with “what was the actual output compared to the intended output?” rather than whether everyone worked hard.
Output has to be read in context. A sales team can sell more at a loss, a factory can raise volume by cutting corners on quality, a development team can ship faster by building technical debt. The book calls these trades, not expanded capacity. Real capacity is higher output with quality maintained, timelines met, resources used efficiently and the system more sustainable.
Accountability comes from criteria set upfront. In the Plan stage the executor defines the expected output, quality, timeline, resources and reliability; the Check stage compares results against them, which the chapter says removes politics and opinion from the assessment. Qualitative work, such as a design decision or a leadership communication, still needs a rigorous standard. The criteria must also be honest: set them so generously that the system always looks successful, and it never improves. Some outputs can be judged in one cycle; others only across many.
